Program Placement Requirement

Thank you for your interest in CTRL Anger – The Anger Control Lab. Please complete this form in its entirety so we can properly assess your needs and determine appropriate placement within the program. Because CTRL Anger is a court-approved anger management program, accurate assessment and correct placement are required to ensure compliance with legal and educational standards.

All information provided will be reviewed carefully and kept confidential in accordance with program policies. Incomplete forms may delay enrollment and placement.

Participation may fulfill court-ordered anger management requirements, subject to approval by the referring authority.
